Our dedicated team of professional staff are committed to providing patients with the highest standards of care. Throughout our patients’ stay, from pre-admission to discharge, each patient will be treated with the utmost respect and dignity. Pre-admission is an important part of your hospital care.

  • EMAIL: Scan only the completed forms (pages 9-13) and email them to admissions@monashhouse.com.au. PLEASE DO NOT SCAN THE ENTIRE BOOKLET.

  • POST/HAND DELIVERY: Remove only the completed forms (pages 9-13) from this booklet and post or hand deliver them to the Hospital Reception: Monash House Private Hospital, Level 1, 271 Clayton Rd CLAYTON VIC 3168. PLEASE DO NOT RETURN THE ENTIRE BOOKLET.

  

Nursing Pre-admission Information

You will be contacted by telephone prior to your day of Admission by our pre-admission nurse. When you are contacted, your medical history will be reviewed with you. Admission details, pre-operative education and discharge plans will also be discussed.

Please take this opportunity to discuss any concerns regarding your admission.

Your Doctor shall notify the Hospital of the date of your procedure/operation and inform you of your day and time of admission. Your Doctor will also explain your procedure or operation and complete the consent form with you.

You will be instructed by MHPH staff / nurses of your time of arrival and nil by mouth time. This means nothing to eat or drink, including water, prior to your operation.

On the day of admission

Please check in at the Hospital Reception located on the 1st floor at the time indicated by your Doctor and you will be directed to the admission area.

Please bring with you into the hospital anything applicable to your admission including:

  • Doctors admission letter

  • Health fund number / details (if applicable)

  • Medicare card

  • Regular medications

  • Pension health benefits cared (if applicable)

  • Pharmaceutical benefits card (if applicable)

  • Relevant x-rays and / or test results

  • Night attire (if overnight stay)

  • Toiletries if overnight stay)

  • Personal articles i.e sanitary pads (if applicable)

  • Method for settling your account (if applicable) visa, eftpos, cash, mastercard

DO NOT (unless your doctor gives you special instructions)

  • Eat or drink anything after midnight for morning surgery

  • Eat or drink anything after 7am for afternoon surgery – (a light breakfast prior to 7am is acceptable i.e tea and toast)

  • Smoke cigarettes or chew gum

  • Wear jewellery. A wedding ring and watch are permitted

  • Bring valuables i.e mobile phone and large amounts of cash

  • Wear make-up or nail polish

Additional Information

  • Please shower on the day of admission before coming to the Hospital and put on clean clothes

  • Wear garments that are comfortable and easy to remove

  • Check with your nurse before informing relatives / friends regarding the time that you should be picked up

Parking

The visitors' carpark is available on site at Monash House Private Hospital and is accessed via Colonel St. Specially marked disabled carparks are available on site. A parking fee applies (Please note, the visitors carpark is owned and operated by an independent company).

Patient pick up / drop off – 3 minute drop off carparks are available directly outside the main entrance of the hospital.

You can catch either the Cranbourne or Pakenham train line to get to Clayton station.

Visiting Hours

Visiting hours on the overnight ward are flexible, however visitors are asked to leave by 8pm for security reasons. Any requirements outside these hours, please discuss with the nurse in charge.

Day Surgery Patients

Monash House Private Hospital features a Day Surgery Unit, which provides a comfortable environment for patients having procedures that do not require an overnight stay in hospital.

 

Before Surgery

  • Bathe or shower

  • Wear loose, comfortable clothing

  • Do not wear makeup or jewellery, other than a wedding ring

  • Unless otherwise instructed, do not eat or drink 6 hours prior to surgery (this includes chewing gum)

  • Do not smoke or drink alcohol for 24 hours prior to admission

  • If you are taking medication and are unsure about whether to take these prior to surgery, please contact your admitting doctor.

 

What to bring

On the day of admission please bring:

  • Any letter(s) from your doctor relating to your procedure

  • Details of Health Insurance Membership

  • Medicare Card

  • Pharmaceutical, Pension or Veteran Affairs entitlement card

  • A list of current medications

  • Cash / Credit Card to pay out of pocket expenses on admission (Excess or Co-payments)

  • X-rays if applicable

  • Glasses and physical aids (e.g. walking sticks, hearing aids)

 

Post-Surgery

  • You will not be able to drive for 24 hours if you have had sedation or a general anaesthetic. Please arrange for a responsible adult to drive you home or accompany you in a taxi.

  • Before you leave the hospital:

    • Our staff will provide you with post procedure instructions including medications, dressings and follow up appointments.

    • Please go via reception to be discharged and finalise the account.

Your Stay

Monash House Private Hospital is comprised of single room only accommodation.  Each room is fully equipped with a telephone and television.  All our rooms feature private ensuites and a range of amenities, designed to provide our patients with a comfortable environment during their stay.

Meals and refreshments are served at the following times:

  • Breakfast: 7.45 – 8.30 am

  • Morning tea: 10.00 – 11.00 am

  • Lunch: 12.00 – 1.00 pm

  • Afternoon tea: 2.30 – 3.00 pm

  • Dinner: 5.00 – 6.00 pm

  • Supper: 7.30 – 8.00 pm

Please discuss any special requirements with your nurse.

Interpreter Services can be arranged where necessary. If you feel this service may be required, please ask the admission staff to initiate enquiries on your behalf.

Telephone for standard local calls are provided free of charge. To access a local number dial “0” and then the number required.

STD & Mobile calls cannot be made from your room phone, please use your own mobile phone or contact your nurse.

Your Discharge

The discharge time for all patients is between 9:00am - 9:30am. Please vacate your room before this time to allow us time to prepare for the next patient.

On discharge, patients can be picked up from the Overnight Ward entrance door on Level 2 or at the car park on Level B1 to meet their assigned transport person. If the patient cannot be collected by this time, they can wait at reception on Level 1 or near the hospital entrance on the Ground Floor.

ATSI Support Officer

Aboriginal and Torres Strait Islander Support Officer

Monash House Private Hospital has a designated Aboriginal and Torres Strait Islander Support Officer, who is also the Director of Nursing and the Compliments, Complaints and Feedback Officer for the Hospital.

 
 

The Aboriginal or Torres Strait Islander Support Officer is available for any person who attends the Hospital to discuss specific care needs, goals and expectations, as is any member of staff throughout the hospital stay.
Monash House Private Hospital welcomes input from Aboriginal and Torres Strait Islander people that will assist us in providing the right cultural support and safe and quality care.
If space is required for Aboriginal and Torres Strait Islander people to consult with family or health care workers within the hospital, please speak with our Support Officer who will arrange a room.
Please advise the Support Officer or any member of staff if you require assistance with translations. Monash House Private Hospital encourages the support of family and friends in ensuring cultural and care needs are met, and that informed consent is made.

Patient & Carer Involvement

We take a holistic approach to your care from pre-admission to discharge.

We encourage family/carer involvement in all aspects of your care. Bedside handover of your care occurs between nurses at changeover of shift times, we encourage your involvement and that of your partner/family/carer at these times.

A communication whiteboard will be located in your room, it outlines your care for the day and tells you the name of your nurse each shift. You and your carer are encouraged to be involved in the filling in of your individual communication whiteboard.

We regularly conduct consumer meetings to get feedback from patients and their relatives on what we can improve.

It is very important that you are well informed about your financial obligations prior to your admission to the hospital.

  • How do I pay?

Privately Insured Patients

Depending on your company and level of cover with your health fund, an out-of-pocket balance may occur for your planned procedure.

Private health insurance policies have 4 tiers: Bronze, Silver, Gold and Basic. With regards to pain management procedures, it is important to note the 2 key categories and where your procedure fits.

  1. Pain management procedures WITHOUT A DEVICE (neurostimulator) will be covered by all 3 tiers EXCEPT Basic or Extras.

    • There could be more out-of-pocket expenses for bronze/silver.

  2. Pain management procedures WITH A DEVICE (neurostimulator) are available to Gold ONLY.

    • Optional to all other tiers

    • CHECK with your insurance company if Neuromodulation devices (stimulator trial or implant) are covered.

Questions to Ask Your Health Insurance

  • Does my policy cover me for this procedure? – You will be provided with the appropriate item numbers.

  • Do I have an “excess” payment on my insurance policy?

  • Are there any co-payments required for each night I will be in hospital?

  • Does my policy exclude some treatments – for example cardiac, orthopaedic or rehabilitation?

  • Are any prosthetic or disposable items used in the surgery not covered by my insurance?

What May Not Be Covered by My Health Fund?

  • Pharmacy (medicines required during your admission and discharge medications)

  • Pathology (e.g. blood tests)

  • Assisting surgeon

  • Other consultants

  • Ambulance transfers

All estimated out-of-pocket expenses are payable before or upon admission.

Going Ahead with Your Procedure

You will be contacted by the various providers confirming rebates or out-of-pocket expenses:

  1. Proceduralist fees - these fees are eligible for a rebate from Medicare which will be provided by billing@metropain.com.au.

  2. Anaesthetist fees (for sedated procedures) - these fees are eligible for a rebate from Medicare which will be provided by another company such as PSJ-A, 03 9059 8045.

  3. Monash House Private Hospital fees or a different hospital fees - theatre/bed fees and consumables. These costs are determined by the type of procedure. There is no rebate on these fees. Monash House Private Hospital can be contacted on 03 8394 0700.

  4. Radiology fees - these fees are eligible for a rebate from Medicare which will be provided by AIIMM, 03 9855 2149.

Self Funded or Un-insured Patients

Un-insured or self-funded patients are required to cover the following fees associated with the procedure at Monash House Private Hospital (MHPH) which includes the proceduralist, anaesthetist and imaging involved. Payment MUST be received prior to or on the day of Admission.

  1. Proceduralist fees - these fees are eligible for a rebate from Medicare which will be provided by billing@metropain.com.au.

  2. Anaesthetist fees (for sedated procedures) - these fees are eligible for a rebate from Medicare which will be provided by another company such as PSJ-A, 03 9059 8045.

  3. Monash House Private Hospital fees or a different hospital fees - theatre/bed fees and consumables. These costs are determined by the type of procedure. There is no rebate on these fees. Monash House Private Hospital can be contacted on 03 8394 0700.

Total Estimates for Your Procedure at Monash House Private Hospital (MHPH)

You will be emailed ESTIMATES by MHPH for the above providers. Please confirm via email or call your doctors’ rooms as soon as possible.

  • You have 2 business days to respond if your procedure is booked within 1 month.

  • You have 5 business days to respond if you are booked more than 1 month away.

Third Party Patients (WorkCover / TAC / DVA)

Total payment (aside from any ancillary charges) must be made on admission unless approval for admission has been confirmed by your insurance company.

Department of Veterans’ Affairs

Department of Veterans’ Affairs (DVA) patients will have their account directly sent to the DVA for payment.

Gold card holders are covered for all care. White card holders are covered subject to approval by DVA.

Workers’ Compensation or TAC

Workers’ Compensation patients need to ensure their Admission has been approved by the appropriate insurance company prior to Admission, and a copy of your approval has been provided to Metro Pain Group and Monash House Private Hospital.

 

How do I pay?

For your convenience, payment of excess/co-payments may be made by cash, EFTPOS, Bank cheques, MasterCard or Visa directly to Monash House Private Hospital (MHPH) at the time of admission. If you have any further questions, please call us on 03 8394 0700.
